Birthday Treats Order Form

$4.00 for lunch
Extra lunch helpings $2.00
$80.00 for a "lunch ticket" (20 lunches)
75¢ for milk (for students who pack their own lunch, otherwise included with lunch purchase)

The lunch menus as well as the meals are prepared by Notre Dame High School.

***Checks for milk tickets should be made to “Notre Dame Nutritional Services”
*** Checks for snack tickets should be made out to St. Joseph Regional School***
